I hereby affirm that I have a current and valid (unexpired) California Drivers’ License and will maintain such so long as I participate as a driver performing UMOW activities.
As a driver for UMOW, I hereby agree that I will maintain automobile bodily injury and property damage liability coverage in the amount of at least $50,000 per accident on any motor vehicle used by me while participating in UMOW activities.
Practices and Procedures:
I understand that I will be trained by UMOW. I will comply with all training procedures and will always practice safety and report any hazards which I observe. I will also make every effort to obey safety precautions provided in writing and/or explained to me.
Volunteer Waiver and Release:
I hereby agree to release UMOW from any and all liability relating to injuries that may occur during my UMOW volunteer activities. I acknowledge the risks involved in UMOW activities, in which I am participating voluntarily. I also fully understand that UMOW does not assume responsibility for, or any obligation to provide, financial or other assistance, including but not limited to medical, health or disability insurance, for any injury, illness, death or property damage sustained by me while participating in UMOW activities. I hereby release and forever discharge and hold harmless UMOW and its successors, assigns, directors, officers, agents, members, and volunteers from any and all liability claims and demands of whatever kind of nature, either at law or in equity, which arise while I am performing my volunteer activities with UMOW.
Photo Release:
I hereby grant and assign UMOW all right, title, and interest in all photographic images and video or audio recordings that are made during my volunteer time with UMOW.
Furthermore, I certify that I have made true, correct, and complete answers and statements on this application.
